How Xealth helped a leading academic health system automatically deliver targeted hypertension education before primary care visits

For patients living with hypertension, timely education can reinforce the importance of blood-pressure management, diet, and the risks associated with uncontrolled hypertension. The health system wanted to make that education more consistent while creating a stronger foundation for conversations with care teams.

Patients living with hypertension may also face comorbidities such as chronic kidney disease or heart failure, making education about managing blood pressure and diet especially important. The health system wanted a scalable way to provide relevant education to patients with hypertension ahead of their primary care visits without adding manual work for clinical teams.

The health system used Xealth to automatically identify patients with hypertension and deliver targeted educational videos before upcoming primary care appointments. The content reinforced the importance of managing blood pressure and diet to help patients understand how those behaviors can affect their overall health, including the risk of developing or worsening related conditions.

By automating delivery at the right point in the care journey, the organization could provide consistent education to a large patient population without requiring clinicians to manually identify patients or send materials individually.

More than 135,000 patients received the educational video bundle through the automated workflow. Among patients who engaged with the content, more than 80% watched 75% to 100% of the videos. By delivering education automatically before an upcoming visit, the health system can reinforce important information without adding a manual outreach step for care teams and give patients an opportunity to arrive prepared for more engaging and better-informed conversations with their providers.
